How Enrollment Works

  1. Book a Free Consult: Schedule a 15-minute introductory visit using the scheduler below. Appointments must be booked at least 2 hours in advance.
  2. Consult Visit: During the consult, I will help determine the appropriate level of care for your needs.
  3. Portal Access: You'll receive a link to our patient portal.
  4. Complete Intake & Schedule: Fill out the required medical history forms in the portal and schedule your comprehensive initial evaluation.
